The following supportive services promote the success of our residents by assisting them in maintaining stable housing, encouraging medical compliance and healthy living, and fostering an atmosphere toward self-sufficiency.
Social Work Case Management assists individuals in developing a personal care plan and connects residents with social, medical, legal, financial, and community resources needed to work toward health and independence.
Medical Case Management provides healthcare assessments, referrals, and medical record and medication reviews. The Medical Case Manager works in tandem with the resident, their doctors, and community medical staff to ensure each resident receives the most appropriate and timely care possible while promoting medical compliance and healthy living.
Substance Abuse Counseling is offered daily on-site to help residents achieve and maintain sobriety.
Home Health Care is provided 24/7 by Resident Care Attendants in the SRO to assist with medication management, take vital signs, help residents keep medical appointments, provide emergency care/alerts, and assist residents with mobility issues and personal care activities.
Medical Transportation helps residents get to and from and improves adherence to individual care plans.
Food/Nutrition Services provides individualized nutritional assessments and meal planning for residents. Individuals who reside in the SRO are provided three well-balanced meals daily. Residents living in the Villages of Samaritan, the Genesis, and Grace Villages are connected to area food banks for food assistance on as needed basis.
Life Skills and Enrichment Programs offers classes and counseling for job training, self-care, money management, adult literacy/GED classes, interview skills, and basic computer training. Also provide recreational and social programs essential to building community, combating boredom, and developing social skills for improved re-integration and independent living.
